INS Sindhuratna. Loss of lives. Very, very sad.

As wife of retired naval officer, fire on INS Sindhuratna followed by death of 2 young officers comes as absolute shock. Worse still, the accidents are occurring too frequently.
Wonder what could be the reason- No accountability, complacency, indifference or plain simple bad luck?
Whatever! It sure spells doom if not taken care of.
